In the following, we will ask you a couple of general questions regarding health and medical conditions.
Please answer the questions truthfully.
If you have been living for an extended period of time in a country considered having a higher risk of tuberculosis, you might be required to undergo a medical examination before a decision is made on your visa application. For which countries this is mandatory, is determined by the Department based on World Health Organisation (WHO) data.
This examination must be undertaken by officially authorised physicians (panel physicians). Unfortunately, this is associated with additional costs not included in our service fee. However, as a Visapath Australia client, we will assist you throughout the whole process, including preparation of the relevant documents and providing contact details of authorised panel physicians in your country.
Please note: The requirement for a medical examination may extend the processing time of your visa applications by several weeks.
If you intend to work or study in a healthcare facility or get yourself treated in a healthcare facility, you may be required to undergo a medical examination before a decision is made on your visa application.
If you intend to work, study, or attend any a training during your stay in Australia as a doctor, dentist, nurse, or paramedic, you may be required to undergo a medical examination before a decision is made on your visa application.

Please answer truthfully and as precise as possible and give a brief statement addressing the following:
In addition to your statement, the Australian authorities may ask you to provide a national police certificate. If the police certificate is in another language than English, an official translation may be requested as well. This is entirely in the discretion of the case officer assessing your application. We recommend, therefore, to apply for your police certificate at your earliest convenience and to submit it proactively (with translation if it contains any records) as soon as you have received it.
If you have been sentenced to more than 12 months' imprisonment, you cannot be granted a Working Holiday Visa.
Please note: Applicants with criminal charges or convictions usually have to expect processing times of several weeks.
We need some additional information. Please briefly explain your function or tasks in the respective service.
If you have worked for more than one organization, please indicate this under "other information" at the bottom of this form.
Please note: Applicants who served in a military force, police force or similar, usually have to expect processing times of several weeks. Therefore, we recommend applying for your Working Holiday visa well in advance.
You have served in a military force, police force or similar but stated that you have not been trained using weapons.
Please note that this questions includes basic training as well. We would therefore ask you to check your answer. A brief indication of the type of the completed training is perfectly sufficient.
Finally, there are only some official declarations left.
